Teeth Cleaning Liability Waiver

Dear Valued Client, Please take a moment to review this Liability Waiver. In order for K9 FlossBoss/Furtastic Paws Inc. to work on your pet, this form must be completed and signed. Please note that any animal may have underlining health issues, which are not the responsibility of K9 FlossBoss/Furtastic Paws Inc., and, accordingly, there are risks when working with your pet. Waiver & Hold Harmless Agreement By signing this agreement, the pet owner agrees to indemnify & hold Furtasticpaws Inc., its principals, technicians, shareholders, officers, directors, employees, and contractors harmless from & against any and all claims, demands, or causes of action, liability, loss, damage or expenses, including legal fees and costs, arising out of or in any way related to the course of any animal teeth cleaning activities. This indemnity & hold harmless agreement shall include, without limitation, any claim, demand or causes of action against Furtasticpaws Inc. or any of its employees, officers, or shareholders for negligence and/or strict liability. By signing below, you agree to release and hold FurtasticPaws Inc, from & against any claims, liabilities, demands, actions or causes of action for any injuries, loss, inconvenience or damages whatsoever suffered arising in any way from the services being provided. __ I understand that K9 FlossBoss is not a Veterinarian & this service is strictly cosmetic & will not cure any underlining illnesses, health conditions, and diseases my pet may have. __ I understand that this service provided by K9 FlossBoss is not to replace Veterinary care. __ I understand that if my pets plaque & tartar build up is severe there can be a chance of infection to my pet’s mouth & or eyes. __ I understand that K9 FlossBoss is not a veterinarian & cannot diagnose or prescribe prescription medication for my pet.__ I understand that my pet is awake during this teeth cleaning procedure & K9 FlossBoss may not be able to clean all plaque & tartar visible in my pet’s mouth. __ I understand that if my pet has loose or wiggly teeth K9 FlossBoss may not be able to clean those teeth.__ Homeopathic remedies maybe used to calm my pet during any procedure. __ I understand that signing this paperwork I release K9 FlossBoss/Furtastic Paw Inc. (including its principals, technicians, shareholders, officers, directors, employees, and contractors) from any harm that may unintentionally happen to my pet. NOTE: Any dog with questionable health, or with possible suspected problems beyond the scope of this cleaning will be referred to see a veterinarian. K9 FlossBoss/Furtastic Paw Inc. is not a veterinarian & this procedure should not be considered a veterinary cleaning but a cosmetic cleaning only. Owners Name: ______________
